Need Camp or School Physical Forms?  When your teen is up to date with Annual Visits, ALL medical reports and physical forms are filled out by Dr. Hipke for FREE.  Just get him the paperwork and he will return it in just a day or so.  This applies to

  • Sports Camp Medical Forms
  • Medical Reports for Insurance
  • Requests for Medical Equipment
  • Family Medical Leave Act Forms (one per patient)
  • Windridge Equestrian Physical Forms

(This offer will not apply to disability claims, certain medical record requests,  life insurance reports nor SSI requests.)

Avoid School Physicals

Annual Visits are great to teach teens about sports safety and nutrition.  With Texas heat, they need to stay hydrated, too.Play ball!

While Dr. Hipke has done thousands of school physicals at area schools, he can perform a much better physical exam and actually hear your teen's heart at the office.

Teens are more comfortable in a private office with a doctor they already know.  Thus, as long as teens are up to date on Annual Visits, he asks them to AVOID SCHOOL PHYSICALS and just get him the papers and he will complete it for FREE.  This is done as a service to your teen so they can avoid multiple 'physicals' in a year.

You can download the current Texas UIL Physical Form here (pdf format),  Please fill out your sections and then bring by the office.  Dr. Hipke will complete his sections and your teen is good to go!
